Transitional Housing Phase I (THP-I)

What is it?

This program  is designed to provide advocacy and support to clients entering second phase of residential programming.

Who is appropriate for referral?

Residential clients:

  • Who have actively participated in emergency services program.
  • Who have completed initial client-counselor contract.
  • Who have established short-term goals.
  • Who have worked to accomplish short-term goals.
  • Who are addressing and attending to immediate needs (i.e. safety, physical and mental health, finances, housing, substance abuse treatment).
  • Who have expressed a desire to continue with participation in Sanctuary’s residential programming (i.e. case management, counseling, groups, guidelines for communal living, etc.).
  • Who are is seeking solutions for self-identified issues.

What is offered?

  • Advocacy
  • On-going case planning
  • Resources and referrals
  • Thorough client history and assessment of past/present issues
  • Counseling
  • Goal-setting and strategies
  • Collaboration with children’s advocates for holistic approach to family advocacy
  • Follow-up services for a period of 3 months after completion.

What are the goals?

  • Decision making
  • Problem solving
  • Progress toward self-stated goals
  • Identification of family needs (communication skills, parenting techniques, recreational ideas, division of roles and responsibilities, etc.)
  • Healing through counseling (supportive, group, and/or therapy)
  • Accountability to meet program expectations

Referrals for THP I will come from advocates in ESP.
